Brain Injury Program Medicaid
The Brain Injury Program provides rehabilitation and life skills services to help Vermonters with a moderate to severe traumatic brain injury, live successfully in community-based settings. This is a rehabilitation-based, choice-driven program intended to support people to achieve their optimum independence and help them return to work.

Adult Family Care Medicaid
Adult Family Care is a 24-hour Home and Community Based Service shared living option for Vermont’s Long-Term Care Medicaid Choices for Care (CFC) Program. This option is typically referred by a Primary Care Physician (PCP) and is available through CFC to participants in the Highest and High needs groups.
Adult Family Care provides participants with person-centered support in a home environment that is safe, family-oriented, and designed to support autonomy and maximize independence and dignity. Adult Family Care is provided in the residence of the Home Provider who provides care and support to no more than two people unrelated to the Home Provider.

HireAbility Vermont is an energized network of employment and job skills specialists, and counselors who are dedicated to breaking down the barriers that have traditionally kept good jobs out of reach of Vermonters with disabilities. For over a decade, they’ve made lasting professional connections between local employers with roles to fill and job seekers eager to lend their talents and expand their career paths.

Adult Education
Vermont's adult education and literacy system offers essential services at locations throughout the state with trained professionals. Personalized instruction is offered in mathematics, reading and writing, essential workplace skills and English for speakers of other languages (ESOL and civics education for immigrants). All educational services are informed and guided by the College and Career Readiness Standards (CCRS) for Adult Education.

Adult Day Application
Adult Day Services support adults with physical or cognitive impairments to stay active in their communities. These services, provided in community-based centers, offer professional health, social, and therapeutic support, along with meals and social interactions. The Adult Services Division (ASD) certifies 14 providers statewide and ensures quality. Funding comes from Choices for Care or Medicaid programs, with private payment options based on a sliding fee scale.
